Our cleft cuties with wider clefts are still normally able to be repaired in a single surgery, without a lip adhesion.
This may be done without taking skin from the columella as in some techniques. The backcut at the depth of the white roll seems to do well long term with the three dimensional crest of the white roll, rather than the flattening that can be seen in a typical Millard style repair as the straight scar contracts (or at least doesn’t grow as much as the surrounding tissue).
Also important is the adequate release of the muscle and reorientation of the fibers, and release of the alar base with lateral nasal wall reconstruction.
We saw him back today doing great at 1 week post-op, and can’t wait to watch him continue to thrive!
