On-table results from Cleft Lip scar revision and rhinoplasty.
The lip was lengthened with Z-plasty of the skin as well as underlying muscle. It was relatively mild, but a difference of 1-2mm can mean a big difference when it comes to the lip.
In this instance, the nasal tip position and asymmetry was helped with asymmetric lateral crural strut grafts using the tripod/quadripod model of rhinoplasty. A slightly longer leg on one side helped to lean the tip away to the other side.
When there is deficient volume, I sometimes use asymmetric alar batten grafts as well.
The nostril symmetry is almost always improved (though rarely ever perfect!).
The septum and nasal bones were straightened and supported as well, with a complement of fixed columellar strut and spreader grafts.

Case Details
Cleft lip repair is traditionally done at about 3 months of age.
Some centers have moved this up to 2-4 weeks, once the child is doing well and gaining weight. There may be some benefits to feeding and weight gain doing this on the earlier side.
