For patients seeking a more feminine facial profile, a number of options are possible. This includes forehead / hairline changes, brow lift and reduction, rhinoplasty, cheek implants or fat grafting, jawline reduction, and more.

I always begin by asking your own goals and priorities, and try to understand each person’s hopes and expectations.

Here, the focus was on her brow bone. A CT scan helps us see the extent of the frontal sinus, and whether the bone can be simply burred/smoothed down, or if it needs to have the outer wall set back, or even some building up of the bone around it to blend the contour.

A combination of burring and a setback was done to achieve a less prominent brow. A fascia graft and scar revision was also done for the nasal dorsum to remove an old scar and soften the upper third of the nose and radix.

Her swelling is coming down well, and we look forward to seeing her long-term results!

Facial feminization has been a rewarding and fun addition to my practice.

The facial surgeries are all in our wheelhouse, including:

Here, we see her at 3 months after:
– Forehead/brow reduction with burring (no frontal sinus setback needed)
– Mandibular contouring with shaving of the angles and of the chin

This helped to soften these areas in a safe and reliable way, with incisions hidden in the scalp and inside the mouth, and no implants required to remain in the body in this instance.

Swelling will continue to improve over the next 6-12 months.

Facial feminization – like all plastic surgeries – is a very personal and personalized procedure.

The goal is usually to help people avoid being misgendered in day-to-day life, though some people with gender dysphoria are trying to match their inner picture of themselves with their outer.

Plastic surgery usually addresses discrete anatomical problems. But a core factor affecting the weight of the physical issue, is always the mental and psychological preparation and work that goes into having a solid, resilient foundation for oneself.

For this family of surgeries, we only care for patients with a strong support system including psychiatric/psychological/therapeutic experts, hormone therapy >1 year with a plateau in visible changes, and all the usual baseline requirements for elective surgeries including overall health and realistic expectations.

Because of all of this prep work, most of our patients coming in to discuss this surgery tend to be very well-informed and self-aware compared to the average patient.

While I do always focus on a patient’s goals, I have evolved to provide a little more direction and suggestions for ways to harmonize each person’s face.

We also have a thorough discussion of the risks and expectations, and do work with insurance to have this covered whenever possible.
