For patients seeking a more feminine facial profile, a number of options are possible. This includes forehead / hairline changes, brow lift and reduction, rhinoplasty, cheek implants or fat grafting, jawline reduction, and more.

I always begin by asking your own goals and priorities, and try to understand each person’s hopes and expectations.

Here, the focus was on her brow bone. A CT scan helps us see the extent of the frontal sinus, and whether the bone can be simply burred/smoothed down, or if it needs to have the outer wall set back, or even some building up of the bone around it to blend the contour.

A combination of burring and a setback was done to achieve a less prominent brow. A fascia graft and scar revision was also done for the nasal dorsum to remove an old scar and soften the upper third of the nose and radix.

Her swelling is coming down well, and we look forward to seeing her long-term results!

Facial feminization has been a rewarding and fun addition to my practice.

Here, we see her at 3 months after:
– Forehead/brow reduction with burring (no frontal sinus setback needed)
– Mandibular contouring with shaving of the angles and of the chin

This helped to soften these areas in a safe and reliable way, with incisions hidden in the scalp and inside the mouth, and no implants required to remain in the body in this instance.

Swelling will continue to improve over the next 6-12 months.

Facial feminization – like all plastic surgeries – is a very personal and personalized procedure.

The goal is usually to help people avoid being misgendered in day-to-day life, though some people with gender dysphoria are trying to match their inner picture of themselves with their outer.

Plastic surgery usually addresses discrete anatomical problems. But a core factor affecting the weight of the physical issue, is always the mental and psychological preparation and work that goes into having a solid, resilient foundation for oneself.

For this family of surgeries, we only care for patients with a strong support system including psychiatric/psychological/therapeutic experts, hormone therapy >1 year with a plateau in visible changes, and all the usual baseline requirements for elective surgeries including overall health and realistic expectations.

Because of all of this prep work, most of our patients coming in to discuss this surgery tend to be very well-informed and self-aware compared to the average patient.

While I do always focus on a patient’s goals, I have evolved to provide a little more direction and suggestions for ways to harmonize each person’s face.

We also have a thorough discussion of the risks and expectations, and do work with insurance to have this covered whenever possible.

Facial feminization is a gratifying operation to reduce unwanted attention, and thus improve comfort in day-to-day situations.

Here, a brow reduction and setback helped to soften the lower forehead and brow region, and to soften and open up the eyes.

In most cases, if we try to reduce the forehead with just burring / smoothing down, then the front of the frontal sinus would totally be burred through, and there would be a big depression in the forehead.

To prevent this, the front wall of the sinus is cut around, the remaining bone is smoothed down, and then the wall set flush with the new position.

If the wall is still too curved – based on the planning – then I may even split the bone across to give a pivot line to flatten out the bone like flattening out an open book sitting face down on a table.

In this case, the mandibular angle and chin were also reduced from incisions inside the mouth.

One of the most common questions is where the incision is placed to reach the forehead and brows for softening.

One option is a bicoronal incision, placed across the top of the head, behind the hairline. This is more hidden, especially for women who wear their hair up and back. However, if the brows are being lifted, the entire hairline is lifted with it and can make the forehead appear taller.

Another option seen here is an anterior hairline incision. It can be more noticeable at first, but tends to camouflage well, and allows the brows to be lifted, while keeping the hairline in a similar area, if not even slightly advanced/forward.

Here, her frontal sinus was set back and forehead and orbital rims burred back. Her nose was smoother to remove the dorsal hump. Fat grafting was also done to the midface, mandibular angles reduced, and chin narrowed as well.

She is seen at one year post-op.
